There are many articles describing the "mistakes" organizations make. As there is no fundamental agreement on what Agile is, they come to different conclusions.

I think some of these are not mistakes, just a bit of misunderstanding. For example 1) The desire to be agile - is exactly another way around. We want to have organizations agile first place and it could be an operational goal. Because the reason why we adopt Agile is to unlock the creative potential of the people, who help us with designing better, maybe faster, and specifically value-oriented operation and delivery. From my experience, the biggest challenge I see is a lack of knowledge (knowledge management & innovation capital) which directly impacts culture (motivation & engagement) and Value stream (plus appropriate org structure). It is not about SW development practices but often about managerial thinking when managers often without realizing it create impediments.

PS: I have never seen any successful and sustainable Agile adoption implemented in other than a top-down way. In 15 years.
